Sports Physical Requirement

A sports physical must be on file in the nurse’s office in order for your student to be able to try out. Forms can be picked up in the Attendance Office or they can be mailed to you. When returning the physical form, please make sure we have all signatures from parents and students. if you are scheduling your student’s sports physical for next year, please don’t schedule before May 1, 2009.

Sports physicals must be turned in on or before the first day of fall sports tryouts. Forms have been mailed to each family.

Sports physicals are required for all athletes annually. While it is recommended that students have their physicals completed by their family physician, we also offer options to Miege families this summer.

Dowload and complete BOTH forms posted below and turn them in to Bishop Miege High School.

STUDENTS CANNOT PARTICIPATE IN COMPETITIONS UNTIL BOTH FORMS ARE COMPLETED AND TURNED IN TO BISHOP MIEGE.
